Admission Policies for Junior High School
1.1 Applicants must pass the admission test and the interview screening for the level applied for.
Note: Admission /testing fee is paid at the Accounting Office.
1.2 Applicants must complete and submit the requirements needed for enrollment / admission.
1.3 Applicants are considered enrolled only after he/she has submitted the required admission or transfer credentials and other pertinent documents and after payment of fees.
For Continuing Students:
a. Fully accomplished application form
b. Philippine Statistics Authority (PSA) National Statistics Office (NSO) Copy of Birth Certificate
c. Four (4) copies of latest (1.5 X 1.5) picture
d. Certificate of Junior High School Completion
e. Qualified Voucher Recipient (QVR) Certificate (for Grade 10 completers from private schools who applied for ESC)
f. Education Service Contracting Certification (ESC) from Junior High School Principal (for ESC Grantees since Grade 7)
g. Learner’s Registration Number (LRN) for Grade 10 completers from public schools
h. Fully accomplished Student Health Record
i. Latest Report Card duly signed by the Principal
j. For applicants who belong to the top 5 of the overall class shall submit a certificate from the Principal regarding class rank/honor
a. Clear photocopy of passport (present original for verification)
b. Four (4) copies of latest (1.5X1.5) ID Picture
c. Copy of ACR/Study Permit
d. Certification of Health Record(issued by Family Doctor or General Practitioner)
e. Transcript of Records or equivalent
f. Police Clearance
